When you bring the cluster online, or during failover, the cluster log records:

00000720.00000bc4::2004/11/11-23:17:31.043 INFO Physical Disk : Online, wait for PnP notification returns 258. 00000720.00000bc4::2004/11/11-23:17:31.043 ERR Physical Disk : Online, volumes not ready. Error: 258.00000720.00000bc4::2004/11/11-23:17:31.043 INFO Physical Disk : Online, setting ResourceState 4.

The above messages indicated that a volume has NOT been brought online, possibly because of multipathing dependencies, filter drivers, or hardware behaviors.

NOTE: If a volume shadow copy fails, the hidden flag would NOT be cleared, causing error 258.

If more than 4 messages in a row are recorded, the cluster log records:

00000720.00000bc4::2004/11/11-23:17:31.043 INFO Physical Disk : Online, returning final error 258 ResourceState 4 Valid 0

NOTE: You may receive the above message during heavy activity, like a migration. It indicates that the disk could not be brought online after 4 attempts. If you cannot bring it online manually, contact your storage vendor, or relevant filter driver vendor.

NOTE: See Server Clusters: Storage Best Practices for Windows 2000 and Windows Server 2003.

NOTE: See New Diskpart.exe commands to reset volume attribute flags on failed snapshot volumes.
