To enter phone numbers with extensions on the contact form, do I need to add a separate field?

You don't need to use a special field. You can just type the extension after the main number (e.g., 800-555-3478 ext 234). Outlook, however, won't include the extension if you use the Call Contact command to try to dial the number.
